Is there a way to avoid rollover fees?

Hello, I have a 401(k) account, and I've been paying exorbitant fees. So my question to you is: How do you avoid rollover fees? Is it possible? If not, I am considering rolling over my 401(k) to an IRA account. Would I have to pay any penalties or fees?
